Our Physical Therapists treat health-related conditions, illnesses, or injuries that limit one’s ability to move or perform basic activities of daily living (ADL). Physical Therapy includes muscle strengthening and retraining, transfer techniques, stair climbing, training in the use of prosthetics, gait training, and adaptive equipment training. Our goal is to restore the highest level of functional independence to each individual.
Speech Therapy is focused on improving communicative abilities, cognitive functioning and swallowing and chewing disorders. After the initial evaluation, we will tailor an individual plan of care. Our therapists will provide treatments in the areas of speech, voice, fluency, and swallowing.
Central Gardens Post Acute Occupational Therapy team works to create an individualized care plan that improves basic motor functions and reasoning abilities. Occupational Therapy focuses on improving functional abilities like getting dressed and bathing, the use of adaptive equipment to perform basic functions, office work, laundry and preparing basic meals. Through the individual care plan custom interventions are created to improve overall function and get back to being you again!
Our experienced team of physical, occupational, and speech therapists work closely with individuals after hospital stays to begin their road to recovery. Around the clock nursing care provides 24-hour care and treatment for a variety of diagnosis and injuries. With motivation and social support from our clinical and therapy teams, our primary goal is recovery and helping you safely return home.
We are proud to offer extensive wound care led by our Wound Doctor and team of treatment nurses. Our MD specializes in wounds and works directly with our nursing staff to organize and conduct weekly wound care rounds for patient consultation and follow up to ensure that wounds are properly cared for and promote maximum healing potential.
We are proud to offer qualified, experienced nursing care to our residents whether your stay is short or long-term. We have registered nurses, licensed practical(vocational) nurses, as well as certified nurse assistants available 24 hours a day to ensure that your skilled nursing needs are met.
We offer an activity program 7 days a week through our Activities Department, to meet the needs aligned with our residents. We understand that mental and physical well being are very important to the recovery process and we tailor our programs to the interests of our current residents. Some of the activities you might see ongoing are social gatherings, exercise classes, artwork projects, iPad games, musical entertainment, karaoke, newspaper and coffee and current event discussions.
Nutrition is key to your recovery. We have a Clinical Dietitian that reviews each diet to ensure a well balanced approach to eating and recovery. This is a vital element to recovery post surgery or hospital stay. They will work with you, your family and the medical professionals to develop meal plans essential to your recovery.
Central Gardens Post Acute is proud to partner with Pacific Inpatient Medical Group (PIMG), the largest hospitalist group in San Francisco County. PIMG physicians provide care at each Sutter Hospital in San Francisco including: Van Ness, Davies, and Mission Bernal.
Physicians from the group caring for you in the hospital will follow you to San Francisco to ensure continuity of your care as you progress towards returning home. This is the most hands-on continuum of care approach available in San Francisco.
Our Social Services seek to improve the quality of life and well being of each resident by helping residents cope with emotional, social, and medical healthcare needs. They help to promote overall well being. They will work with you and your family on a discharge plan to discuss next steps after care and coordinate home health services to transition home with you. They also facilitate care conferences, support groups, and provide counseling services for residents and family members.
